Guided HealthCARE is a clinical based Nurse Care Coordination and Case Management company, fully staffed with Registered Nurses that support chronically ill patients, their families, loved ones and the medical team as patients navigate the healthcare system. We assist our patients by coordinating all aspects of their care and by helping our clients/families make informed decisions to obtain the best health care options possible.

 

We have been providing care for 15 years and continue to grow.  We are well established in the community and have long-standing and trusted relationships with the medical community and partners throughout the Bay Area.  We continue to recruit experienced Clinical Nurse Case Managers to our team, including full-time and part-time (15-30 hours per week) opportunities. The RN in this critical position will be responsible for all aspects of Clinical Care Coordination and Case Management for their own client caseload, consisting of patients with a wide variety of acute and chronic healthcare needs across the lifespan. The role of a Clinical Nurse Case Manager combines working out in the field, visiting adult patients in their residence or clinical location, attending physician visits/appointments, and working both remotely and from a home office to manage the many aspects of care coordination.

 

Major Responsibilities: Some of the primary responsibilities of working with Guided HealthCARE include the following:

  • New consults and admission of new patients

  • Development of holistic and comprehensive plans of care

  • Ongoing patient physical and psychological assessments

  • Medication management, in-home assessments, patient necessitated nursing tasks

  • Professional communication between the comprehensive medical/care team, patient and loved ones for continuity of care

  • Ongoing patient, family, and caregiver education

  • Referrals and recommendations based on patient needs

  • Support and coaching for office visits, including all appropriate documentation

  • Regular review and maintenance of electronic medical records

  • Participation in ongoing primary care conferences

Job Requirements:  Applicants for this position must have the following skills and experience:

  • Current Registered Nurse licensed to practice in the state of California

  • Graduate of an approved and accredited nursing program

  • Preferred 5 years nursing experience, including acute care, but will consider a strong candidate with less experience

  • Strong patient assessment skills

  • Strong organizational skills and ability to prioritize tasks quickly and effectively

  • Current BLS certification

  • Outstanding written and verbal communication skills

  • Ability to be a strong yet professional and tactful representative for our patients

  • Ability to work independently and efficiently

  • Ability to use iPhone as a professional technology tool

  • Computer literate with strong MS Office 365 skills

  • Knowledge in working with digital formats including Clinical Based EHR/EMR Systems

  • Valid CA driver’s license, current automobile insurance and access to a reliable vehicle

  • Proof of personal RN liability insurance

  • RN Case Management/Home Health/Hospice/Community RN experience a plus

  • Certification in Case Management (i.e., through CMSA) a plus
